Why the right assessment is so important

Many of those affected live with unexplained symptoms for years before they are diagnosed with lipoedema. The pain is often dismissed as a consequence of being overweight.

Yet lipoedema is a condition in its own right. If you know the signs, you will receive suitable treatment sooner – and you can do a lot yourself: a stable weight, compression and exercise relieve the symptoms. Lipoedema does not necessarily progress.

What lipoedema feels like

Physical warning signs

These symptoms distinguish lipoedema from ordinary excess weight – and should make you take notice.

Weight loss barely changes the proportions

The upper body becomes slimmer, but the imbalance with the legs or arms remains

Heavy legs in the evening

Feelings of heaviness and tightness increase over the course of the day, often with the sensation that the legs are swelling

Pain on pressure and touch

Even light pressure on the thighs or upper arms causes disproportionately severe pain

Pain without a cause

The affected areas also hurt at rest – without pressure, a knock or exertion

Frequent bruising

Many of those affected bruise easily – often reported, but not a reliable sign on its own

A feeling of tightness

The skin feels full and taut, as if it were being pushed from the inside

Visible features

How to recognise lipoedema by its appearance

Alongside the symptoms, there are characteristic outward signs that point to lipoedema.

Always symmetrical

Lipoedema always affects both sides equally – unlike lymphoedema, which often occurs on one side only.

Slim torso

The torso is never affected: the upper body stays slimmer while the legs and arms increase disproportionately in circumference.

Slim hands & feet

The most typical feature: the fat distribution ends abruptly at the wrists and ankles.

Fat pads on knees & upper arms

Fat pads on the inside of the knees and the back of the upper arms are typical – equally on both sides.

Stage 1, 2 and 3

The three stages of lipoedema in detail

The usual classification describes how the skin and fatty tissue look and feel – not how severe the symptoms are. Since October 2025, liposuction has been covered by German statutory health insurance at all stages (for patients insured in Germany).

Lipoedema stage 1

What you see: The skin is smooth and the fatty tissue is evenly thickened. The hips and legs look larger than the upper body – often dismissed as a “normal problem area”.

What you may feel: Pain on pressure and touch, feelings of heaviness and tightness – often already considerable, even though little is visible from the outside.

Lipoedema stage 2

What you see: The skin surface becomes uneven and wavy, and the circumference increases more noticeably. A “cuff” can form above the ankles, while the feet stay slim.

What you may feel: The same symptoms as in stage 1 – sometimes stronger, sometimes weaker. The inner thighs rub against each other more often.

Lipoedema stage 3

What you see: Markedly increased fatty tissue with overhanging folds of tissue, for example on the inside of the knees and on the thighs.

What may be added: Skin irritation due to friction, misalignment of the knees such as knock knees, and restricted mobility.

The stage says nothing about the pain

According to the current guideline, the stage classification should not be used as a measure of how severe the condition is. Severe pain is just as possible in stage 1 as mild pain is in stage 3. Whether surgery is an option depends on your symptoms.

No automatic progression – and no stage 4

Lipoedema does not necessarily move from stage 1 to stage 3: with a stable weight, it stays the same for years in many women. There is no separate “stage 4”; the term usually refers to lipoedema with additional lymphoedema.

Liposuction covered by German statutory health insurance – at all stages

In July 2025, the Federal Joint Committee (G-BA) decided: since October 2025, German statutory health insurers have covered the cost of liposuction – regardless of the stage. The decision was based on a large comparative study that showed equal benefit for all three stages. This applies to patients insured in Germany. The complicated application procedure no longer applies.

All stages

Until October 2025, cover was limited to stage III. Since then, patients with stage I and II also benefit from statutory cover.

In line with G-BA requirements

We perform operations covered by statutory health insurance at NATIONALKLINIK Wertheim, a hospital included in the German state hospital plan – using the tumescent technique, as required by the G-BA.

What you need for statutory cover – we take care of it

6 months of conservative therapy, a BMI of no more than 35, the two-doctor principle (diagnosis and indication for surgery by different doctors) – sounds complicated? It doesn’t have to be. Diagnostic methods

How lipoedema is diagnosed

The path to a confirmed diagnosis involves specialised examination methods. The diagnosis required by German statutory health insurance is made by a specialist approved for this purpose – we will tell you who is eligible.

Pinch test

The doctor carefully pinches the tissue. In lipoedema, this causes disproportionately severe pain – an important first indication.

Duplex ultrasound

Ultrasound can rule out vein disease and other causes. It cannot prove lipoedema.

Written report

You receive a complete medical report with the diagnosis, stage and treatment recommendation – the basis for all further steps.

When should I see a doctor?

If you notice several of the typical symptoms – especially pain on pressure and touch, a feeling of heaviness, and legs or arms that stay noticeably larger than the upper body despite weight loss – you should see a specialist in angiology, dermatology or physical and rehabilitation medicine, or a doctor with additional training in phlebology. The earlier lipoedema is recognised, the sooner you can do something about the symptoms.

Can my stage get worse?

It can, but it does not have to. According to the current guideline, lipoedema is not fundamentally progressive: with a stable weight, it stays the same for years in many women. The most important trigger for a deterioration is weight gain; hormonal changes such as pregnancy or the menopause can also play a role. A stable weight, compression and exercise help.

Which stage do I have?

Only a specialist can determine the stage reliably. What matters is the surface of the skin, the texture of the tissue and the extent of the fat distribution. The stage, however, says nothing about how severe your symptoms are. We examine you and discuss your findings with you. The confirmed diagnosis required by German statutory health insurance is made by a specialist approved for this purpose – we will tell you who is eligible.

Will health insurance cover the cost of surgery?

For patients with German statutory health insurance: yes, since October 2025 liposuction has been covered at all stages. The requirements are: 6 months of conservative therapy, no weight gain during this time, a BMI of no more than 35, and the two-doctor principle (diagnosis and indication for surgery by different doctors). With a BMI between 32 and 35, the waist-to-height ratio (WHtR) must also be appropriate for your age. Do I have to deal with the applications myself?

No. Since October 2025, patients with German statutory health insurance no longer need to submit an individual application to their health insurer. You don’t have to worry about a thing.

How do I tell lipoedema from cellulite?

Cellulite consists of dimples in the skin caused by the structure of the connective tissue – it does not hurt and is not a disease. Lipoedema is a disease of the subcutaneous fatty tissue on the legs or arms: it hurts on pressure and touch, and the legs or arms are noticeably larger than the torso.

Is there a lipoedema stage 4?

Not as a separate stage. “Stage 4” usually refers to lipoedema that has been joined by lymphoedema – known as lipo-lymphoedema. This is a second condition with its own treatment. If your feet or hands swell as well, you should have this checked by a doctor.

From which stage does lipoedema surgery make sense?

The stage does not decide this. According to the guideline, the decision for liposuction should be based on your symptoms: it is an option if the pain persists despite consistent conservative treatment. Since October 2025, German statutory health insurance has also paid regardless of the stage, provided the other requirements are met.
