Lipoedema fat does not respond to diets

This is what you need to know: the diseased lipoedema fatty tissue is resistant to conventional diets and exercise. No matter how strictly you count calories – these fat cells will not disappear through weight loss.

But losing weight can still be important. It improves your overall situation, takes strain off your joints and lymphatic system – and, for many patients insured in Germany, it is the key to statutory insurance cover. To have liposuction covered, your BMI must be no more than 35.

Why a BMI of no more than 35 matters so much

Since October 2025, German statutory health insurers have covered liposuction for lipoedema (for patients insured in Germany) – but only under certain conditions. BMI is one of them. We perform these insurance-covered operations at our NATIONALKLINIK Wertheim – preparation and aftercare remain at the location near you.

BMI requirement

For statutory insurance cover, your BMI must be no more than 35 – between 32 and 35 only if your waist-to-height ratio (WHtR) is appropriate for your age. If your BMI is higher, medically supervised weight loss is needed first – and this is exactly where Weight Doctors comes in.

No weight gain

Important: your weight must not increase during the 6 months of conservative therapy. Weight Doctors helps you stabilise your weight or reduce it in a targeted way – depending on your starting point.

Documentation

The course of your weight must be documented by a doctor. With the 3D body scan you have complete records for your files – all digital and available at any time.

How GLP-1 therapy works

GLP-1 receptor agonists (semaglutide, tirzepatide) have transformed the treatment of obesity. They work on several levels:

Here you will find answers to the most important questions about weight loss with lipoedema.

What BMI applies to lipoedema surgery?

For lipoedema surgery to be covered by German statutory health insurance, your BMI must be no more than 35. If it is between 32 and 35, your waist-to-height ratio (WHtR) must also be appropriate for your age – calculate it with our WHtR calculator. In addition, your weight must not have increased during the six months of conservative therapy. If your BMI is above 35, medically supervised weight loss comes first, for example with Weight Doctors. As a self-pay patient, the insurer's BMI requirements do not apply – we will discuss in your consultation whether surgery makes sense for you.

Why should I lose weight if lipoedema fat doesn't respond to diets?

That is a fair question. Lipoedema fat itself really does not respond to calorie reduction – that is exactly what makes this condition so frustrating. But many patients also carry ‘normal’ excess weight in addition to their lipoedema, and this can be influenced by losing weight. Also, a BMI of no more than 35 is a requirement for cover by German statutory health insurance – between 32 and 35 only if your waist-to-height ratio (WHtR) is appropriate for your age. Losing weight also takes strain off your joints and improves your lymphatic system.

What is GLP-1 and how does it work?

GLP-1 (glucagon-like peptide-1) is a hormone produced naturally in the gut after eating. It regulates blood sugar and satiety. GLP-1 medicines (semaglutide, tirzepatide) mimic this hormone and enhance its effect. They reduce hunger and slow down stomach emptying, leading to an average weight loss of 15–20% of body weight.

Is GLP-1 therapy suitable for me?

GLP-1 therapy is suitable for adults with a BMI of 30 or more, or a BMI of 27 or more with weight-related conditions. Certain pre-existing conditions may rule it out. Your individual suitability is assessed during a consultation at Weight Doctors. As a lipoedema patient, you can be referred to the Weight Doctors team directly through Lipo Doctors.

What are the side effects of GLP-1 therapy?

Typical side effects at the start are mild gastrointestinal symptoms such as nausea, a feeling of fullness or diarrhoea. These are usually temporary and ease within the first few weeks. At Weight Doctors, the dose is increased particularly slowly over 16–20 weeks to keep side effects to a minimum. Your doctor monitors you closely.

How much does the Weight Doctors programme cost?

The costs vary depending on the type of treatment (GLP-1, gastric balloon) and its duration. Unlike for patients with diabetes, German statutory health insurance does not cover GLP-1 medicines used purely for weight loss – this is a self-pay service. After your consultation you will receive a transparent cost estimate. Payment in instalments is possible.

How long will it take for my BMI to reach 35 or below?

That depends on your starting weight. With GLP-1 therapy, an average of 1–2 kg per week is realistic. If you need to lose 15 kg, for example, you should expect it to take 3–6 months. You can start conservative lipoedema therapy at the same time – so you don't lose any time.

What happens after I have lost the weight?

Once you have reached your target weight, GLP-1 therapy can be slowly tapered off. The eating habits and exercise routines you have established in the meantime help you maintain your weight. Important: your weight must not increase during the 6 months of conservative therapy before lipoedema surgery. That is why we continue to support you after the active weight-loss phase.

Can I have surgery as a self-pay patient without reducing my BMI?

Yes. If you have lipoedema surgery as a self-pay patient, the BMI requirements for German statutory insurance cover do not apply. You can start surgical treatment straight away. We are happy to advise you on which path is better for your individual situation.
